I do not believe I have been exposed to someone with a suspected and/or confirmed case of the /COVID-19.
I have not been diagnosed with COVID-19 and not yet cleared as non contagious by state or local public health authorities.
I am following all CDC recommended guidelines as much as possible and limiting my exposure to the COVID-19.
COVID-19 Studio Policies:
Students must wear a face mask at all times within the building and within the studio, even in the restroom.
Students need to apply the hand sanitizer before entering and while exiting the studio.
Students will maintain a safe distance from others at all times.
Students must carry their own bottle or cup for drinking water.
No one is allowed in the lobby; parents and dancers must wait outside or in their cars.
Students with a temperature above 100.4 will not be allowed into the studio.
Our instructors and staff will:
Clean/Disinfect frequently touched surfaces thoroughly and at regular intervals.
Wash/Sanitize our hands before and after each session.
Wear face masks at all times.
Maintain a safe distance at all times.
Accommodate a limited number of students in each class to ensure physical distancing.
Schedule classes so as to allow time for cleaning/disinfecting surfaces and allow students to finish and leave the studio before the next set of students arrive.
Provide sanitizers in the reception area and other locations within the premises.
Make-Up Classes There are no refunds for cancelled or missed classes. Make-up classes are available at no additional cost. If you would like to attend a make-up class, please speak with your dance instructor.
Snow Policy There are no refunds for classes canceled due to weather. If classes are canceled due to inclement weather, you will be notified by email. We will also post any class cancellations on our Facebook and Instagram pages. Be sure to follow us to stay connected!
HOW DOES DWA COLLECT AND USE MY INFORMATION? When you register for classes, we collect contact information such as guardian names, addresses, phone numbers, emergency contacts, e-mail address and student information such as student names, birthdays, medical conditions, and your comments. WHO HAS ACCESS TO MY INFORMATION? When you register, you are required to enter an email address and a password. The combination of that e-mail account and password is used to ensure that only you have access to, and only you have the ability to modify, your personal information through DWA's registration system. DWA will access your personal information and/or share it with third parties only for the following purposes: (i) as needed for studio business; (ii) if required to do so by law or to comply with legal process; or (iii) to protect the rights, property, or personal safety of the public.
